Custom Form Select Issue - Showing Double Items

When using the Custom Form Select option. It seem that the select items are showing twice once published. Anyone else having this happen? Is there a solution to prevent this?

Hey @tristan! Check your site settings for a duplicated script. I see it being added twice here.

@Support-Luis Thanks! It was also in the custom code section. Not sure how I overlooked it. That solved my issue. Greatly appreciated!

I am having the same issue although I double checked and the script is not duplicated. What could be another reason for the selections being duplicated?
